Medication

Any student who is required to take medication at school is required to bring the medication to the nurse's office with a Medication Order Form filled out by a student's guardian and the physician who administered it. Click here for a copy of this form.

Any student who is required to take medication on a school sponsored field trip should fill out a Field Trip Medication Authorization Form, which can be found by clicking this link.

Medical Notification

Any student who has a medical condition that should be noted by the school nurse should send documentation of that condition from a physician.  The school nurse does not automatically share that information with the staff unless the student's guardian grants permission to do so. Food Allergies

If the school nurse should be notified of a food allergy, please download and complete the Food Allergy Action Plan.

 

 

 Physical Examination and Immunization Requirements

State law requires certain immunizations and physical examinations for all public school students.

 Height and Weight Collection

New regulations from Mass Department of Public Health requires schools to collect height and weight measurements of students in grades 1, 4, 7 and 10.
