Registration information

In order to enroll your child at the Coakley Middle School, the following documents are required:  

  • Birth Certificate

 

  • Passport (if coming from another country)

 

  • School records   
    • Report cards
    • MA Transfer Card
    • Discipline record
    • Attendance record
    • IEP (if Special Education student)
    • All testing to support the IEP (if Special Education student)
    • 504 plan if applicable

 

  • Proof of Residency

  • Updated Health Records

  • Norwood Public Schools Personal Data Sheet   pdf
    • Complete the attached document

 After ALL forms are completed, please call the guidance office to schedule an appointment to meet with a counselor.  This process can only be completed by the legal guardian (please bring a valid ID). 

Sending schools can fax documents to the Norwood guidance fax; however, a meeting with a counselor cannot be arranged until all documents are received.
