If enrolling in one of our medical family plans for the first time, please send up a copy of your marriage certificate and any children(s) birth certificates with the enrollment forms.

  • If a member has started treatment prior to the effective date of the new High Plan we would prorate the orthodontic benefits. We would need the following information from the dental office: Total Case Fee of the Orthodontic; Type of Orthodontic Treatment; Date of Banding; Months of Treatment.
