go to “Health Services”, “Immunization Information”, then “Immunizations - Grades 7-12 Entry Requirements”. We recommend that you contact your child’s physician prior to the start of school to verify that all immunizations and physical examinations are up-to-date as listed on the “Entry Requirements” document.
Please note that the school district must comply with the state law. Therefore, it may be necessary to exclude your child from school if you do not provide the required documentation of full compliance to Katherine McDonough RN, school nurse, before the first day of school on September 4.
