Please take a few minutes to review the information, particularly the forms titled “Medication Administration” and “TLC Parent Permission Form”. If your child requires medications during school hours and/or should be included on our confidential list of students with specific health needs, please complete either or both of these forms before the start of school on September 4, 2014 and return them to Katherine McDonough RN, School Nurse.
